Complete Your Patient Paperwork

Once your appointment is booked, save time at the office by completing your health history, insurance, and HIPAA forms through our secure Denticon portal — the same HIPAA-compliant system we use to manage your chart. Everything you submit goes directly into our practice management system.

Step-by-Step Instructions

We recommend completing the Denticon paperwork on a computer or tablet rather than a phone — the forms are easier to read and the digital signature field is simpler to use. Here is what to expect:

    Click the “Fill Out Patient Forms” link above

    You will be taken to our secure Denticon patient portal. The URL starts with patientregistration.denticon.com — you should see a lock icon in your browser's address bar.

    Enter your personal information

    Name, date of birth, address, email, and emergency contact. If you are registering a child, enter the child's information — parent/guardian fields come next.

  3. 3

    Provide insurance details

    Upload a photo or enter the member ID, group number, and subscriber name from your dental insurance card. If you are self-pay, simply skip this section.

  4. 4

    Complete your health history

    Be thorough here — medications, allergies, recent surgeries, heart conditions, diabetes, and pregnancy all affect dental treatment planning.

  5. 5

    Sign and submit

    Type your name as a digital signature on the HIPAA and consent forms, then click submit. You will see a confirmation screen — you do not need to print anything.
